WSD Positive / Probable Cases and Quarantine Reporting Form
Please complete this form if you are the parent and/or guardian for a child who has tested positive for COVID-19, in the past two weeks, is considered to be probable for COVID-19 in the past two weeks, or are currently under quarantine due to exposure with an individual who has tested positive or is probable for COVID-19.
A separate form should be submitted for each school-aged child.
In accordance with the Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) and the Health Insurance Portability and Accountability Act of 1996 (HIPAA), all student educational records and health information collected in through this form are protected and will be kept private. -
The Western School District has created and made available a report concerning the progress made in meeting the educational goals contained in its Extended COVID-19 Learning Plan not later than February 1, 2021, for goals its expected would be achieved by the middle of the school year and not later than the last day of school of the 2020-2021 school year for goals the Western School District expected would be achieved by the end of the school year.
